1. No the trample only becomes a charge when a model survives the attacks. So a Mumak doesn't charge in the traditional way.
2. I cannot remember the exact rule but I believe they can
3. Yes he must or else lose his movement just like anyone else
4. Yes but not out of combat as they must be able to move to cast

I am pretty sure you cannot use "Shielding" when riding a mount.
