In WOTR do you have to field the mounted version to get his Legendary formation/monster version with the special movement? Or can i just use the foot version?

The model shown on the picture is the mounted one. Or just put the foot version on a cavalry sized base.

It never actually says the size that the base must be, but for fairness' sake, just blue-tack your foot version onto a cavalry base (since he is stated as being on a horse, lol).

Yeah it depends really, if your opponent is over the top padantic. Then it would have to be mounted, but as everyone else has sed putting him on a cavalry base would be fine :D
